Why are my questions not showing up on my pre-screen form?
So that the questions in your pre-screen request form show up, first you need to save your public profile in the ''Public Profile'' section and your questions on the ''Pre-screen'' section.

Setting up your public profile
1. From your main dashboard, click on "Referrals."
- Go to the "Public Profile" section.
- Click on the section(s) of your public profile you want to set up or edit.
- After editing the section(s), make sure each section checkbox you want to show is checked, and click on "Save profile."
Setting up your pre-screen form
1. Go to the "Pre-Screen" section.
2. To add a new question under a section, click on the "+" sign below each section.
3. To ensure that a section(s) appears in your consultation request form, make sure to check the boxes next to each section.
Your pre-screen form will save automatically.
