How can I administer a rating scale while I am composing a note?

Premium subscribers are now able to administer rating scales directly from a note! Simply click the link "Fill out a Rating Scale" which is found in the drop-down under "Actions".

1. Under the text box in a note, click on the "Action"  drop-down and select "Fill out a Rating Scale".

2. The rating scales will pop up - simply choose one from the drop-down.
