Where does the employee assistance program number go on the HCFA form?

1. Employee assistance program numbers can be entered in the patient's/client's insurance form by going to Charts>Billing>Insurance Info.

2. They can also be entered right before submitting the claim on Charts>Billing>All Transactions>I want to...>Insurance>Submit.

3. On the patient's/client's insurance form, enter the employee assistance program number in the ''Prior authorization number (23)'' field.

4. Here you can also enter prior authorization numbers.
