How do I get the provider practice info to pre-populate on the lab request form?

To update the "Provider Practice Info" on the lab request form, please follow these simple steps:

1. Choose your selected patient in ''Charts", then click on "Labs''.

2. Under the "Clinician Practice Info" section, click on the "Account>Settings" link as shown below.

3. This opens in a new tab, the "Accounts" section, and into the "Practice Info" tab. Here, click on the Tax ID tab and enter your tax ID by clicking on the "+ Add Tax ID" link as shown below.

4. Click on the "Save" button.

Note: The "Branch Info" tab next to the "Tax ID" tab needs to be completed for the information to pre-populate. 

5. Directly to the left of the "Practice Info" tab, click on the "Licensure" tab. Click on "NPI info" and enter your rendering NPI by clicking on the "+ Add Rendering NPI" link.

6. Click on the "Save" button.

7. If you need to update your location, click on the "Edit" link under Practice Info>Office Info.

8. Enter your new location information, then click on the "Update" button.

9. Go back to the ''Charts>Labs'' section and refresh the page. From the drop-down, select the branch with the information that you would like to have auto-populated on your lab request form.
