How do I add an addendum to a signed note?

Whether you've left out a detail, or just thought of more later, you can add an addendum to a note that has already been signed. 

Follow these steps to add an addendum to a note: 

1. In the patient's/ client's chart, go to Entries>All Entries.

2.  At the top-right side of the desired note, click the arrow to access the drop-down menu and click "Add addendum".

3. Enter the additional information, and click "Confirm".

4. You'll see the addendum at the bottom of the note now!

Please note: For legal reasons, it is not possible to have a note changed in any way after it is signed. However, if you need to send this note out, and do not want to send the note and addendum together, you can copy and paste your note into a new note, with the correct info and then mask the original note.
