How do I send a rating scale (symptom measure) to a patient/client?

Here's how you can send a rating scale to a patient/client in Luminello.

1. You can use the ''Shortcuts'' menu by selecting the chart and clicking on ''Send form'' from the ''Go to'' dropdown.

1. From the chart's "Dashboard" section, go to the "I want to..." drop-down, and choose "Send Form."

2. Check the box next to the rating scale (s) you want to send to the patient/client, and click on "Send to portal."

3. You will receive a confirmation message that the rating scale has successfully been sent. Once sent, an unofficial chart entry will be recorded, and the patient/client will get an email notification to fill out the rating scale (s).
