How do I create a custom form using the form builder?

You can build custom templates, questionnaires, and administrative documents with the form builder. Read on to learn how, or watch a video here: 


1. From your main dashboard, click on "Practice."

2. Select "My Form Builder" from the "Practice" drop-down.

3. Select the form you want to create from the "Form Type" drop-down.

  • "General Admin Form" - This form is used for administrative documents. 
  • "Questionnaire - Intakes" - This form asks patients/clients questions in preparation for an intake.
  • "Questionnaire - Follow Up" - This form asks patients/clients questions in preparation for a follow-up.  
  • "Note Template - Intakes" - This form is used to compose intake notes with custom fields that our basic intake notes don't have.
  • "Note Template - Follow Up" - This form is used for composing SOAP notes with custom fields that our basic SOAP note doesn't have.
  • "Note Template - Free text" - This form is used to compose free text notes with custom fields that our basic free text notes don't have.

3. Add a name to your custom form under "Form Title."  4. To start building your custom form, drag and drop elements under "Question types" into the form area, and click "Edit" to set up your questions/answers
