How to send an Intake form to the patient/client portal?

Clinicians can send their custom-made intake/follow-up questionnaires to the patient/client portal to fill out electronically, but only once they have created their Luminello portal accounts. Read on to learn how. 

1. You can use the ''Shortcuts'' menu by selecting the chart and clicking on ''Send form'' from the ''Go to'' dropdown. 

2. You will land on the patient's dashboard, and all your active forms will be displayed on the screen. Check the box (es) next to the form (s) you want to send.

3. Scroll down to the "Send To" field, where you can select the patient/client and any family member from a pick list. 

4. Click "Send to portal."

5. You will see a success banner, and an unofficial entry will be added to the chart for each form sent.
