Oh no, I signed the note with the wrong date. What do I do?

Because notes are legal records, it cannot have the wrong date deleted once a note is signed.  Here are two workarounds:

  • Add an addendum. Click here to learn how.
  • Clone the original note, make sure the date is correct, then "hide" the original note. Here's how:

1. Go to "Charts."

2. Choose your patient/client by typing his name.

3. Click on Entries>Compose Note.

4. Select a previous note from the "Use previous" drop-down.

5. Change the date on the cloned note to the correct date under "Date to display in chart."

6. Scroll down to the bottom of the note, and click on the "Sign" button.

7. Then, go to the "All Entries" tab, and locate the note you would like to hide. On the drop-down, click on "Hide."

8. Enter a comment in the box (this is required); then click ''Confirm''.

9. You will see the header "Original Entry Unavailable" and your comment instead of the note.
