How do I create an admin form?

These are the steps to create an admin form:

1. From your main dashboard, click on "Practice."

2. Select "My Form Builder" from the "Practice" drop-down. 

3. Pick "General Admin Form" as the "Form Type" from the drop-down. 

4. Add a title to your custom form under "Form Title."

5.  To start building your custom form, drag and drop elements from under "Question types" into the form area, and click "Edit" to add your text. 

6. Click "Save Form" to save your custom form.
