What are the best workflows for filling out an intake and creating a progress note?

We recommend two approaches for clinicians to fill out an intake and create a progress note. Read on to learn more:

Option One: Via a Custom Intake Questionnaire


Clinicians can send their personalized intake questionnaire to the patient or client portal for electronic completion, provided the individuals have created their Luminello portal accounts.

  1. Access the ''Shortcuts'' menu, select the appropriate chart, and click on ''Send form'' from the ''Go to'' dropdown list.


  2. This action will direct you to the patient's dashboard, where all your active forms will be displayed. Check the box(es) adjcent to the form(s) you wish to send.


  3. Scroll down to the "Send To" field, where you can choose the patient/client and any family member from a pick list.


  4. Click "Send to portal."


  5. Upon successfully sending the form, you will see a banner and an unofficial entry for each sent form will be added to the chart.

    Once the patient has finished and submitted the intake questionnaire, the clinician can then incorporate this data into the intake note.

  6. Locate the completed questionnaire in the patient's ''All Entries'' section, click the dropdown menu, and select "Create Intake note."


  7. A new Intake note pre-filled with the questionnaire results will be displayed on your screen. You can make further edits as needed.

Option Two: Via a Custom Intake Template


As an alternative to an intake questionnaire, clinicians can utilize a custom intake note template. This template features a similar format and types of questions:

  1. From your chart's dashboard, select "Compose Note" from the "Entries" dropdown menu.


  2. You can opt for the readily available custom intake note template named “Luminello Sample Intake Template”, or you can choose a template you've created yourself using our form builder.


For both methods, you can conveniently create a follow-up note and clone the note from the calendar for swift note completion in the future. We also suggest setting up a few snippets or customizing the pre-loaded ones to expedite your note-taking process.
