How do I fill out the spreadsheet to activate E-labs with Change Healthcare?

Here's how to fill out the spreadsheet to activate e-labs with Change Healthcare:

  1. Fill out the mandatory fields (highlighted in red) on the ''Clients'' tab to provide details about your practice:
  • Channel Partner: Enter "Luminello."
  • Client name: Enter your name or your practice's name.
  • Address: Enter your practice address, city, state, and zip code.
  • Phone: Enter your phone number.
  • Fax: Enter your fax number.
  • Tax ID: Enter your Tax ID number.

  1. Fill out the mandatory fields (highlighted in red) on the ''Providers'' tab to provide details about you and any other provider in your practice:
  • Last name: Enter your last name following by the last names of any other providers in your practice, if applicable.
  • First name: Enter your first name following by the first names of any other providers in your practice, if applicable.
  • NPI: Enter your NPI following by the NPI of any other providers in your practice, if applicable.
  • Clinician User: If you are a solo practice, select "Y," otherwise, select "N."

  1. Fill out the mandatory fields (highlighted in red) on the ''Users'' tab with your assistant (s) information, if there are any in your practice:
  • Last Name: Enter the last name for any assistant (s) in your practice, if applicable
  • First Name: Enter the first name for any assistant (s) in your practice, if applicable
  • Mother's Maiden Name: Enter the mother's maiden name for any assistant (s) in your practice, if applicable.

  1. Fill out the mandatory fields (highlighted in red) on the ''Lab Information'' tab:
  • Lab Name: Enter the name of the lab.
  • Lab Account Number (s): Enter the account number for the lab. If you don't have an account number, you must request one with the lab (s) before completing this form.

5. Download the completed form as a Microsoft Excel file, and share the form with us via email to start your e-labs integration.
