How do I add a comment explaining why a medication was stopped?

Here's how you can add comments to a medication explaining why you decided to stop it:

1. On the "Rx" section of a patient/client's chart, click the "Stop" icon next to the medication you want to stop. 

2. Select a reason for the pick-list or type in your own by clicking "Other."

3. Click "Stop."
