Tell me more about custom forms and templates

Clinicians can fully customize questionnaires for their patients/clients to fill out through their portals and electronically send back to the clinician. The questionnaires can be used for Intake and Follow Up/SOAP notes. Check out this guide to learn how to create a custom form.
