Why can't my patients access my pre-screen form?

Make sure patients/clients can see your pre-screen form by running this troubleshooting checklist:

  • Your public profile must be set up and viewable. Click here to learn more about how to set up and show your public profile. 
  • Your pre-screen must be set up and viewable. Click here to learn more about how to set up and show your pre-screen form.
  • Existing patients must be logged out of their portal account (not just by clicking the "x" in their browser).  Ask them to click "log out," then try again.
